Missed a Zepbound dose? Here is what to do

The rules are simple — but the timing matters

If you miss your scheduled Zepbound injection, the rule is: if it has been less than 4 days (96 hours) since your missed dose, take it as soon as you remember. If it has been 4 days or more, skip the missed dose and wait for your next scheduled day. Never double-dose. Note: the window for tirzepatide is 4 days, slightly shorter than semaglutide's 5-day window.

In simple terms:
  • Less than 4 days (96 hours) late: take it as soon as you remember
  • 4 days or more late: skip it, wait for your next scheduled dose
  • Never double-dose
  • The window is 4 days — slightly shorter than Wegovy's 5-day window
  • One missed dose will not reset your progress
  • Multiple missed doses: contact your prescriber about re-titration

The rules

Three scenarios, three clear answers.

Within 4 days (96 hours)
Take it now

Inject as soon as you remember. Resume your regular schedule the following week. For example, if you inject on Tuesday and remember on Friday (3 days), inject Friday and go back to Tuesday next week.

4 days or more past your usual day
Skip it entirely

Wait for your next regular day. Taking a late dose too close to your next one risks overlapping drug levels, which can worsen GI side effects.

Missed 2+ consecutive doses
Call your prescriber

You may need to drop back one or two dose levels and re-titrate. Jumping back to 10mg or 15mg after a break is a common cause of severe nausea.

Decision flow

Step through this to figure out what to do right now.

1
When was your last injection?
Count the hours since your scheduled dose

The cutoff is 96 hours (4 days). This is shorter than semaglutide's 5-day window.

2
Has it been less than 4 days (96 hours)?
Yes → inject now

Take the missed dose immediately. Resume your regular schedule the following week.

3
Has it been 4 days or more?
Skip → wait for next scheduled day

Do not inject. Wait for your next regular day. Do not try to compensate by injecting sooner.

4
Have you missed 2+ weeks in a row?
Call your prescriber

Re-titration may be needed to avoid severe side effects when restarting.

Never double-dose
Whether you are on a weekly injection or a daily pill, taking two doses to make up for one missed dose increases the risk of severe nausea, vomiting, and — if you also take insulin or sulfonylureas — dangerously low blood sugar. One missed dose is always better than a double dose.
