Foundayo titration schedule

Simple 4-step ramp from starting to maintenance dose

Last reviewed: April 2026Every claim linked to source

Foundayo titration takes about 12–16 weeks from start (3mg) to the maximum dose (36mg), with one dose increase every 4 weeks. Unlike the Wegovy pill, there are no food timing restrictions at any dose.

In simple terms:
  • Starting dose: 3mg. Doses increase every 4 weeks: 6mg → 12mg → 36mg
  • Total time to reach 36mg: 12–16 weeks (3–4 months) following the standard schedule
  • No food timing rules at any dose — take it anytime with breakfast, lunch, or dinner
  • Side effects usually peak during weeks 2–6 at each dose, improve by week 8
  • You can stay at any dose if weight loss is good — no requirement to reach 36mg

Why gradual dose increases?

Why gradual increases matter
Gradual dose increases give your body time to adapt to the medication and minimize side effects. Clinical trials show that starting low and stepping up every 4 weeks reduces nausea and dropout rates compared to faster escalation. Most people tolerate it well because of the slow ramp.

Week-by-week timeline

Weeks 1–4
3mg
Your first dose

Take any time of day, with or without food. Mild appetite reduction. Mild nausea in 10–15% of people. Some people feel nothing at first. By week 4, appetite improvement usually noticeable.

Weeks 5–8
6mg
Appetite increases

Appetite reduction increases. Nausea may peak around week 5–6, then improve. About 15–20% experience nausea at this dose. Weight loss usually visible by week 8 (2–3 lbs typical).

Weeks 9–12
12mg
Building effect

Stronger appetite suppression. Nausea continues but usually less than weeks 5–8. About 20–25% report nausea. Weight loss accelerates (3–4 lbs typical in this period). GI side effects possible but mild.

Week 13+
36mg
Maintenance — maximum dose

Maximum appetite suppression. Nausea usually mild or resolved. About 25–30% report any nausea. Weight loss continues at about 12.4% average. This is where you stay long-term if pursuing maximum weight loss.

During your titration

If side effects are manageable

Step up on schedule every 4 weeks. Your appetite suppression at that dose is stable and side effects usually improve significantly by week 8.

If side effects are rough

Stay at your current dose for 6–8 weeks before trying to increase. Extra time allows your body to adjust more fully and side effects to improve before you increase.

Why not go faster?

Foundayo has only 4 doses, making titration faster than semaglutide or tirzepatide. The 4-week interval is still standard. Slower increases (6–8 weeks per dose) are fine if side effects are an issue. The goal is to reach a dose where weight loss is good and side effects are manageable.

Nausea during titration

When nausea happens:
  • Nausea usually starts 2–4 days after a dose increase
  • Peaks around days 5–10 at the new dose
  • Improves significantly by days 15–21
  • By week 8, usually improves substantially
What helps:
  • Eat smaller meals (4–5 small instead of 3 large)
  • Avoid fatty, greasy, and spicy foods
  • Stay hydrated with small sips throughout the day
  • Eat slowly and give your stomach time
  • Try ginger (tea, candies, or supplements)
  • Do not skip meals — nausea is worse on empty stomach
  • Stay at a dose for 5–6 weeks if nausea is bad
When to call your doctor:
  • Severe nausea that prevents eating or drinking
  • Persistent vomiting (more than once a day)
  • Inability to keep fluids down
  • Nausea lasting beyond 8 weeks at a dose

Can you slow down or pause?

Yes — your titration schedule is flexible.

Option 1: Extend each dose interval

Stay at a dose longer than 4 weeks (6, 8, or 12 weeks) before increasing.

Option 2: Skip a dose level or stop early

Stop at your goal dose — if you reach 12mg and weight loss is good, ask your doctor to keep you there.

Option 3: Stop at a dose that works

Pause at your current dose indefinitely if weight loss is adequate.

Option 4: Pause and resume later

Pause for a month while you adjust, then resume when ready.
