Weight often returns — this is biology, not failure

People who tend to consider this:

  • You want a pill with no food or water timing restrictions
  • You want to avoid injections entirely
  • You prefer a once-daily pill over weekly shots
  • You are looking for an affordable GLP-1 option (~$149/month)

This may not be right if:

Absolute contraindicationUse with caution
  • You want maximum possible weight loss (injections produce more)
  • You have cardiovascular disease and want proven heart benefit (semaglutide has SELECT data)
  • You have a personal or family history of medullary thyroid cancer
  • You are pregnant or planning to become pregnant

Weight loss — what trials show

In the ATTAIN-1 trial (72 weeks), people taking orforglipron at the highest dose lost an average of 12.4% of their body weight — that's roughly 30 lbs for a 240 lb person. About 6 in 10 people lost at least 10%. The key advantage: unlike oral Wegovy which requires a 30-minute fast before eating, Foundayo can be taken any time with no food restrictions.

About 1 in 10 people in trials did not respond significantly to treatment. If you have been on treatment for 12–16 weeks without meaningful weight loss, it is worth discussing this with your doctor.

These are averages from clinical trials — individual results vary. Trials compare the drug against a dummy pill, not against other weight loss drugs.

How this compares

Average weight loss in clinical trials. Not a ranking — individual results vary.

Orforglipron (Foundayo)you're here12.4%(~27 lbs)
Semaglutide pill (Wegovy)16.6%(~37 lbs)
Semaglutide injection14.9%(~33 lbs)

Averages from clinical trials at highest approved dose. Not everyone achieves these results.

Oral Wegovy produces more weight loss but requires 30-min fasting. Foundayo trades some efficacy for complete flexibility on food timing.

What to expect — week by week

Week 1–2
Appetite begins to change

Many people notice reduced hunger or feeling full sooner. This varies between people.

Week 4–8
~3% lost
Weight loss becomes noticeable

Most people start to see measurable weight loss during this period.

Week 12–16
~6% lost
More significant results

Side effects often settle around this point. Measurable weight changes are becoming more visible.

Month 6–9
~10% lost
Peak weight loss phase

This is when most of the weight loss typically happens. Side effects usually settle.

Month 12+
~12.4% lost
Plateau and maintenance

Rate of loss slows as your body approaches equilibrium.

What happens if you stop?

Weight regain after stopping is expected, consistent with other GLP-1 drugs. In ATTAIN-MAINTAIN, participants who switched from injectable GLP-1 to orforglipron maintained their weight loss — suggesting the drug effect is comparable to injections for weight maintenance.

Weight trajectory — during treatment and after stopping

Based on: ATTAIN-MAINTAIN

Most of the lost weight returns within a year of stopping — this is the body returning to its defended weight, not a failure. Long-term use or maintenance therapy can help preserve results.

Frequencies from ATTAIN-1. Most stomach and gut side effects are mild-to-moderate and peak during the dose increase period (first 12–16 weeks).

Important cautions
  • Do not use if you have a personal or family history of medullary thyroid carcinoma or MEN 2
  • Report persistent severe abdominal pain — may indicate pancreatitis
  • Tell your doctor about all medications — dose adjustments may be needed for some diabetes drugs

This is not a complete list. Your doctor can advise based on your full medical history.

Hair thinning

Some people notice hair thinning or increased shedding around months 3–6 of treatment. This is usually caused by the physical stress of rapid weight loss on the body — a temporary condition called telogen effluvium — rather than the medication itself. In most cases, hair regrows on its own within 6–12 months. If hair loss is severe or persistent, speak to your doctor.

Muscle and lean mass

No published body composition substudy for orforglipron. As a GLP-1 agonist, the lean mass profile is expected to be similar to injectable GLP-1 drugs — proportional to total weight lost.

Detailed body composition data (DEXA scan) has not been published for this drug. The general pattern for GLP-1 medications is roughly 70–80% fat and 20–30% lean mass.

Resistance training and high-protein intake are the two most evidence-based ways to protect lean mass during medicated weight loss.

How to take it

  • Take one tablet daily — any time of day, with or without food. No fasting window required.
  • Starting dose is 0.8 mg. Your doctor will increase it gradually every 4 weeks through several dose levels up to 17.2 mg.
  • Store at room temperature. No refrigeration needed.
  • Do not crush, chew, or break the tablet — swallow whole.

How it works

A small-molecule (non-peptide) GLP-1 receptor agonist. Unlike peptide-based GLP-1 drugs (semaglutide, liraglutide) that are large proteins requiring injection or strict fasting rules, orforglipron is a small organic molecule that absorbs through the gut wall without food interference. It activates the same GLP-1 receptors in the brain and pancreas, reducing appetite and slowing digestion.

Cardiovascular & other benefits

Cardiovascular outcomes data pending. Blood pressure and lipid improvements observed across Phase 3 trials. 91% of prediabetic participants reached near-normal blood sugar vs. 42% placebo in ATTAIN-1.

  • No food or water restrictions — take any time of day
  • Available at $149/month self-pay (lowest dose) or $25/month with commercial insurance
  • Can maintain weight when switching from injectable GLP-1 (ATTAIN-MAINTAIN trial)
  • 91% of prediabetic participants reached near-normal blood sugar (ATTAIN-1)
  • Medicare Part D coverage expected from July 2026 at ~$50/month

Community insights

These are personal experiences shared in public online communities — not medical advice.

The main excitement is that this is a pill you can take without planning around food — unlike oral Wegovy which requires 30 minutes of fasting.

r/antiobesitymedication·Discussed when FDA approval was announced

The 12.4% weight loss is less than weekly tirzepatide or semaglutide injections, but the pill convenience and no food restrictions could be a game-changer for people unwilling to inject.

r/Zepbound·Comparative discussions about oral options

At $149/month self-pay it matches oral Wegovy on price but without the fasting rules. For people who can't do the 30-min window, this changes things.
