Contrave

Contrave dosage — the four-week step up

From one tablet a day to two, twice a day

Last reviewed: September 2026

Contrave (naltrexone and bupropion extended-release) builds up over four weeks: one tablet in the morning in week 1, one morning and one evening in week 2, two morning and one evening in week 3, and two morning and two evening from week 4 onward. That final dose is 32 mg of naltrexone and 360 mg of bupropion a day.

Your step-up schedule

The label adds one tablet a week until you reach two tablets twice a day at the start of week 4.

  1. Week 11 tablet AM

One tablet in the morning, no evening dose. Tablets are swallowed whole — the label says not to cut, chew or crush them.

  2. Week 21 AM + 1 PM

An evening tablet is added, so the daily dose is now split across two times of day.

  3. Week 32 AM + 1 PM

The morning dose doubles. The label says never to take more than two tablets at one time.

  4. Week 4 onwards2 AM + 2 PM

The maintenance dose: 32 mg naltrexone and 360 mg bupropion a day. The label says higher total daily doses are not recommended.

    Maximum dose

This is Table 1 of the FDA label (Dosage and Administration 2.1). The label ties this gradual build-up to seizure risk: it lists escalating the dose gradually, splitting it into two daily doses, never taking more than two tablets at once, and avoiding high-fat meals among the steps that reduce that risk. Response is evaluated after 12 weeks at the maintenance dose, and the label says to stop Contrave if at least 5% of starting body weight has not been lost.

Starting dose vs maximum dose

The maximum dose is the highest approved dose — it is not the same as your maintenance dose.

Starting dose

1 tablet

Your first dose — the lowest to let your body adjust

Maximum dose

4 tablets

The highest approved dose — not everyone needs to reach it

Contrave has a single target dose rather than a range. The FDA label builds to two tablets in the morning and two in the evening, and says total daily doses greater than 32 mg/360 mg are not recommended. Lower daily doses are used when kidney or liver function is reduced, or alongside certain other medicines.

Weight loss at each dose

The label does not report weight loss by dose for Contrave, so there is no dose-by-dose breakdown to show here. See what the trials found at the full dose →

When your doctor might change your dose

Your dosing is flexible and tailored to your needs.

Reduced kidney or liver function

The label caps the dose at two tablets a day — one morning, one evening — in moderate or severe impairment.

Taking clopidogrel or ticlopidine

These raise bupropion exposure, so the label caps Contrave at two tablets a day.

Twelve weeks at the full dose

The label says to stop Contrave if less than 5% of starting body weight has been lost.
