Xenical & Alli

Xenical and Alli dosage — 120 mg vs 60 mg

One capsule with each meal that contains fat

Last reviewed: September 2026

Orlistat has no step-up schedule. Prescription Xenical is one 120 mg capsule three times a day, taken with each main meal that contains fat — during the meal or up to one hour after it. Over-the-counter Alli is the same idea at 60 mg, with no more than three capsules a day. If a meal is missed or has no fat, both labels say the dose can be skipped.

Your daily dose

There is nothing to titrate. The dose is fixed and tied to your meals rather than to a week number.

Xenical (prescription)

120 mg

One capsule three times a day, with each main meal containing fat — during the meal or up to one hour after it. The label says doses above 120 mg three times a day have not been shown to provide additional benefit.

Alli (over the counter)

60 mg

One capsule with each meal containing fat. The Drug Facts label says not to take more than 3 capsules daily.

A meal with no fat

Skip the dose

The Xenical label says that if a meal is occasionally missed or contains no fat, the dose can be omitted.

Multivitamin

At a separate time

Xenical asks for a multivitamin containing fat-soluble vitamins, taken at least 2 hours before or after orlistat, such as at bedtime. Alli says to take a multivitamin once a day at bedtime.

Weight loss at each dose

The label does not report weight loss by dose for Xenical & Alli, so there is no dose-by-dose breakdown to show here. See what the trials found at the full dose →

When your doctor might change your dose

Your dosing is flexible and tailored to your needs.

A meal with no fat in it

Both labels allow the dose to be omitted when a meal is missed or contains no fat.

Tempted to take more

The Xenical label says doses above 120 mg three times a day have not been shown to add benefit; Alli says no more than 3 capsules a day.

A meal high in fat

The label says gastrointestinal effects increase when orlistat is taken with a diet or a single meal high in fat.
